AUHSD Medication Form  - All medication on campus requires medical authorization from a medical provider, as well as a parent's signature.  This includes all over-the-counter medication.  We encourage all non-emergency medication to be stored in the health office for dispensing.  

 

Asthma Action Plan Form  - If your child requires Asthma medication on campus   

 

Food Allergy and Anaphylaxis Emergency Care Plan Form - For allergies requiring emergency medication administration if exposed or ingested. 

 

Seizure Action Plan Form - For school guidelines on management of seizures during school activities.  

 

Diabetic School Care Plan Form - For diabetic management during school activities.  Please complete and attach a copy of your Diabetic Medical Management Plan from your provider 

 

Migraine Action Plan Form - For migraine and severe headache symptom management during school activities.  Please complete this form with your provider.  If your student needs to carry medication with them for immediate treatment of symptoms, please have your provider also complete our AUHSD Medication Form (above). 

 

Doctor's Office Visit Form  - Please complete with your health care provider for any medical visits/conditions requiring support from the health office on campus. 

IMMUNIZATION REQUIREMENTS 

Please provide a copy of your student's immunization record to the school registrar/ school nurse for review.  

Your child must be current in their vaccinations to attend school.

MEDICAL FORMS

If your student has a medical condition that will require adult support at school or medication to be administered, please go to the Medical Forms tab to download the appropriate forms for your medical provider to complete. 

• If your child has a fever. When a child has a temperature of 100 or more, they are often both infectious and also really not feeling well. You cannot truly assess temperature if kids have taken Tylenol or ibuprofen, so make sure you do not check when those medications are on board. Students should be afebrile (fever-free) for at least 24 hours without medication before they return to school.

 

• If your child feels ill enough that they cannot participate in class or focus on learning activities.

 

• If your child is contagious (pink eye, strep throat, flu). Please keep your child home in this case. They will recover better and faster, and are less likely to spread their illness to other students and teachers. If your child needs antibiotics, they should NOT return to school until they have been on antibiotics for at least 24 hours. Students should bring medical documentation (a note from doctor) indicating they have been evaluated and treated, with any pertinent information about their current health status.

 

• Vomiting and/or diarrhea. Your child should not return until they have no vomiting or diarrhea for at least 24 hours.

 

• Sometimes there is not a clear line between when your child should come to school and when s/he should stay home. Any of the above are when students should definitely stay home from school. If you take your child to the doctor, please turn in medical documentation with an update on their condition.
